The Cook County Department of Animal and Rabies Control is partnering with Evanston Animal Shelter to offer free 1-year rabies and and microchips. Clinics are available to Cook County dogs and cats.

Waiver Statement:

Vaccine and Chip Clinic Consent Agreement 

I am the owner, or agent, for the animal(s) enrolled in the Vaccine and Chip Clinic and have the authority to provide consent for services rendered at the clinic that is occurring on 9/27/2025. Therefore, I authorize the services selected to be performed as indicated by my electronic registration. I am aware of the potential benefits and risks of vaccinating my pets. I release Evanston Animal Shelter Association and it's agents of any liability.  

Clinic Guidelines 

One cat per carrier is required. Dogs must be on a leash at all times. If you do not have a carrier or leash, please contact the Evanston Animal Shelter at (847) 866-5082 for assistance in securing one before the vaccine event.

Photo Release Form 

I hereby grant Evanston Animal Shelter Association the permission to use my likeness in a photograph in any and all of its publications, including but not limited to all Evanston Animal Shelter Association's printed and digital publications. I understand and agree that any photograph using my likeness will become property of Evanston Animal Shelter and will not be returned. I acknowledge that since my participation with Evanston Animal Shelter Association is voluntary, I will receive no financial compensation. 

I hereby hold harmless and release and forever discharge Evanston Animal Shelter Association form all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on the behalf of my estate have or my have by reason of this authorization.
